Adoption Decision: Submission of this form does not guarantee adoption of any animal. The adoption decision is at the sole and exclusive discretion of D&R Cat Rescue. 3. Legal Age: You affirm that you are of legal age to enter into this adoption agreement. By signing below, you indicate your understanding and acceptance of the terms outlined in this disclaimer and application. CAT ADOPTION CONTRACT Terms and Conditions Please initial next to each statement to indicate your understanding and acceptance of the following terms of the contract. 1) Responsibilities of Adopter: I agree the cat is being adopted as a companion pet, not for experimental research or alternative purposes. * I agree to provide proper food, water, shelter, and medical care for the adopted cat. * I will ensure that the cat receives regular veterinary check-ups and vaccinations as recommended by the veterinarian * I will not declaw the cat. * I will keep the cat indoors at all times, except when supervised in a safe outdoor environment if the adopter deems appropriate and safe. * I will provide a safe and loving environment for the cat, free from abuse, neglect, and harm. * 2) Return Policy: If, for any reason, I am unable to keep the cat, I will not surrender the cat to any other individual, shelter, or organization without providing 48 hour notice and the consent of Daniel and Rachel Cat Rescue, Inc. * 3) Health Guarantee: To the best of Daniel and Rachel Cat Rescue, Inc.’s knowledge the cat is in good health, unless otherwise notified, at the time of completion of this contract. If the cat is found to have medical/health/physical issues after completion of the adoption process, it is the adopter’s responsibility to provide adequate and timely vet care. * 4) Cat Temperament/Behavior: The Adopter acknowledges and understands that while every effort is made to assess the temperament and behavior of the cat prior to adoption, Daniel and Rachel Cat Rescue, Inc. cannot guarantee the temperament, personality, or behavior of the cat after adoption. Cats, like humans, may exhibit different behaviors in different environments and situations. Factors such as past experiences, socialization, and individual personality traits may influence the cat's behavior. * 5) Liability: Daniel and Rachel Cat Rescue, Inc. is not and will not be liable for any acts of the pet once ownership of the pet has been transferred. * 6) Transfer of Ownership: Upon signing this contract, ownership of the cat is transferred from the care of Daniel and Rachel Cat Rescue, Inc. to the individual completing this form. * 7) Adoption Fee: The Adopter agrees to pay the adoption fee of $150/cat or $250 for two cats upon signing this contract. This fee helps to cover the cost of medical care including but not limited to spaying/neutering, vaccinations, and other expenses incurred by Daniel and Rachel Cat Rescue, Inc. * I understand that the adoption fee is final and that if the cat is return to Daniel and Rachel Cat Rescue, Inc. for any reason the adoption fee will not be returned. * I understand that additional expenses may be incurred for veterinary care and other necessities after transfer of ownership and that these expenses will be the responsibility of the Adopter. * 8) Additional Terms: Accuracy of Information: I certify that all information provided in this adoption form is true and accurate to the best of your knowledge. * Legal Age: I affirm that I am of legal age (18 years old or greater) to enter into this adoption agreement. * Signature: * By submitting this adoption form, you acknowledge and agree to the above terms and conditions.
